POSITION SUMMARY: Provides a variety of support services to the homeless, near-homeless, and/or in crisis; provides linkage to resources and services; provides financial assistance for eligible persons; conducts public relations and fundraising activities; and completes program documentation accurately and timely. Provide support services through the HOPE, Veteran's RRH program, Thrives, HART, and Stability Assistance. Assists with screening of walk-ins, phone calls, etc. to ensure referrals to the appropriate programs are made.
ROLE AND RESPONSIBILITIES
- Responsible for completing the agency intake form and inputting it into Clarity
- Provide ongoing support to veterans and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
- Maintain a non-judgmental attitude toward clients and other CNCAP staff
- Ensure clients are assessed and determine possible assistance through resources
- Create family personal plans to develop short and long-term attainable goals with individuals
- Ensure files are complete with the necessary paperwork for veteran families participating in the program
- Adhere to all CNCAP-related policies and procedures
- Promote the program and agency in a positive and professional manner
QUALIFICATIONS
- High school diploma or GED required (Bachelor's degree in human services or related field preferred)
- Valid driver's license
- 1-3 years of experience in human services or related field

SCHEDULE: Monday-Thursday: 7 am - 5 pm or Monday-Friday: 8 am - 4 pm
REPORTS TO: Housing Programs Coordinator
JOB STATUS: Full-Time, 40 hours per week
PAY: $16.50 - $18.25 (depending on qualifications)
