We are expanding our team to serve the needs of our community in the Lake of the Ozarks, Missouri area! Central Ozarks Medical Center (COMC) is excited to announce an opening for a Manager of Network Development and Population Health!

Responsibilities: The Manager of Network Development and Population Health is a key member of the Leadership Team that assists the Chief Network Development and Population Health Officer to build and maintain community/partner relationships, development, and implementation of health center programs/initiatives/fundings and support the broader Population Health team.
Areas of Responsibility: Major areas of responsibility include but are not limited to program/initiative development and tracking, patient in-reach/outreach program development, implementation, tracking; DRVS data validation, reporting, and support community organizations and partners.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Participate and cultivate community partner relationships to ensure mutually beneficial relationships that further the efforts of supporting our patients and broader communities.
- Assist with identifying and implementing programmatic efforts that support current and future funding opportunities.
- Track and submit weekly, monthly, and quarterly data needed to satisfy funding opportunities.
- Support team members with improving workflows that meet grant/program/initiative requirements.
- Represent Network Development and Population Health on a cross departmental leadership team to ensure integrated and collaborative patient care.
- Assist with staffing levels, hiring, and onboarding team members who fit and support the mission of COMC.
- Complete Data Validation in Azara DRVS to assist with accurate and representative reporting.
- Provide oversight and support for patient in-reach and outreach with a strong focus on patient engagement initiatives.
- Coordinate with school districts on outreach efforts in collaboration with operational teams.
- Aid team members in identifying and obtaining appropriate training.
- Coordinates onboarding of new Network Development and Population Health Team members.
- Other duties as assigned.
Qualifications Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or experience: Bachelor’s degree or equitable professional experience preferred.

Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups and/or Board of Directors.
Mathematical Skills: Ability to work with accounting concepts, develop and follow department budget(s).
Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
Other Skills and Abilities: Supervisory skills, strong training skills and excellent customer service skills. Outgoing personality with strong professional acumen. Strong communication skills that include diplomacy and straight forward instruction. Excellent interpersonal skills and empathy towards patients. Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This includes moderate physical effort consisting of occasionally lifting or moving over 35 pounds of weight.
Travel Requirement: To fulfill the responsibility of the position travel to all COMC locations will be required. The Employee should have a valid Missouri driver’s license. Overnight travel may also be required in specific areas of training, conferences, etc.
