We are expanding our team to serve the needs of our community in the Lake of the Ozarks, Missouri area! Central Ozarks Medical Center (COMC) is excited to announce an opening for a Re-Entry Navigator for the Waynesville Courthouse/Richland Clinic!

Responsibilities: The Re-entry Navigator will support justice-involved individuals in achieving successful reintegration by addressing barriers and connecting them to essential resources. They will coordinate care for all healthcare needs, ensuring continuity and access to services beginning 90 days or less before release. This position will focus on fostering stability, reducing recidivism, and promoting overall well-being through personalized care coordination and community partnerships.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Community Health Worker
- Provides support to individuals through coordination of support services such as transportation, housing resources, and benefit enrollment (Medicaid, SNAP, etc.).
- Advocates for eligible clinical services and connecting patients to care, including after care follow-up and progress monitoring.
- Will assist with PRAPARE forms.
- Help assist patients/community members with SDOH determinants.
- Track and submit data on a weekly and monthly basis to satisfy funding requirements.
- Build upon existing relationships with other community agencies and form new partnerships in order to best assist community residents.
- Maintains accurate patient records in the electronic health records system.
- Participate in training classes and meetings as necessary.
- Other duties as assigned.
Qualifications Requirements:
- Strong oral and written communication skills.
- Ability to work effectively and professionally in a fast-paced environment.
- Knowledge of and ability to work and engage with the uninsured, under-served and under-represented populations.
- Must have intermediate computer skills and be able to type at least 40 wpm.
Education and/or experience: Minimum of a High School Diploma is required. Experience with providing customer service is a plus. Experience with data tracking is a plus.
Language Skills: Ability to read, analyze, and interpret financial reports. Ability to respond to common inquiries or complaints from customers and regulatory agencies. Ability to effectively present information to top management.
Mathematical Skills: Ability to work with accounting concepts such as income and expense statements, budgets, and controls on cash such as into and out of COMC for deposit to the bank.
Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions.

The employee must occasionally lift and/or move up to 50 lbs. Travel Requirement: To fulfill the responsibility of the position travel to all COMC locations may be required. Employee must have valid Missouri driver’s license and availability of a motor vehicle.
