SUMMARY:
The QIDP is responsible for ensuring that quality services are provided to all individuals in the Residential Program. and ensuring each plan is developed and supports Person Centered Thinking and Planning. The QIDP will help to lead teams in creating meaningful activities and valued outcomes for persons supported in the areas of community integration; socialization; recreation, volunteerism and mindful motivation. Promote, research and provide activities that further enrich people’s lives, while providing options in both the program setting and community environments
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Responsible for chairing and coordinating of quarterly, annual and special reviews.
Prepare minutes of all review meetings; summarize reports presented by other members of the Interdisciplinary Team.
Monitor changes and discrepancies that are reviewed and discussed during Interdisciplinary Team Meetings.
Ensure that all recommendations, changes and discrepancies are addressed.
Observe individuals, review data, progress of program and discuss observations with appropriate Interdisciplinary Team Member.
Ensure consistency among internal and external programs and disciplines.
Ensure that all records are current, and accurate.
Assess ADL needs for all individuals and develop programs addressing those needs.
Monitor all programs and make necessary revisions on timely basis.
Ensures that services provided by the DSPs are properly documented and adhere to all OPWDD data collection regulations to ensure proper billing of services render
Support DSPs in increasing their knowledge and ability to capture service and experiences provided via training and modeling
Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches.
EDUCATION AND/OR EXPERIENCE:
Bachelor's Degree in Human Services required /Masters Degree preferred, minimum of two year of work experience, working with the developmentally disabled and/or dual diagnosed population.

Pay: $63,000.00 - $65,000.00 per year
