About the Transitional Living Program
Chambliss Center for Children’s Transitional Living Program provides stable housing and personalized case management for young adults aged 18-21 who have aged out of traditional foster care and chosen to participate in the Extension of Foster Care (EFC) services in Tennessee. Our program offers over two dozen fully furnished, single-occupancy units equipped with washer/dryer, utilities, and internet access—all at no cost to the residents. Participants in the program must stay enrolled in secondary or post-secondary education or maintain employment, in line with EFC requirements. While residents enjoy a high degree of independence, the Transitional Living Specialist works closely with them to ensure they acquire the confidence, competence, and resources needed to live independently.
Responsibilities
- Provides case management services in coordination with agency team members and other service providers.
- Ensures that all record keeping is maintained, is current in its completion, and is timely.
- Participates in the development, implementation, and regular review of an Individual Service Plan for each young adult.
- Participates in all scheduled meetings regarding services for the young adults.
- Coordinates transportation needs of the young adults.
- Identifies and engages with community resources for young adults including educational, therapeutic, employment, and recreational opportunities.
- Provides experiential life skills training including individual and group sessions. This includes, but is not limited to: financial management, maintaining personal health needs, housework, shopping, meal preparation, job skills, and transportation.
- Assists with developing transition plans for youth entering and exiting the program.
- Provides regular oversight for each property belonging to youth on caseload and ensures that maintenance needs are met.
- Performs other work-related duties as assigned.
Qualifications
- Bachelor’s Degree in Social Work, Psychology, or a related field.
- At least one year of social work and case management experience or working with youth in a similar age range.
- Must maintain a valid Driver's License and an acceptable driving record.
- Must be able to clear a background/fingerprint check.
- Excellent written and verbal communication skills.
- Demonstrated crisis intervention skills.
- Adaptability, flexibility, and creative problem solving skills.
- Able to work independently as well as part of a team.
- Ability to build trusting, supportive relationships while maintaining professional boundaries.
