Health Navigator Specialist

Chestnut Family Health Center is seeking a skilled Health Navigator in our Belleville, IL facility. As a health navigator, you will gain expertise to help develop your passion in social work, psychology, counseling, and other behavioral health fields.

Responsibilities:

• Facilitate patient access to quality primary and behavioral health care.
• Provide clinician support using social work and psychology theories and models to reduce symptoms from DSM-5 diagnosis.
• Screen clients for issues related to Social Determinants of Health and make service referrals.

The estimated salary for this position is around $55,000 - $65,000 per year, depending on experience. To be eligible for the position, you should have a minimum of 10 years' experience working in the human services field, healthcare or another advocacy role, or a Bachelor's degree with 5 years' experience.
