Shift: Monday - Friday from 830am - 5pm
Job Summary
Responsible for program management, i.e. planning, implementing, and coordinating the delivery of active treatment services to all residents, and for assuring that treatment programs are consistent with federal and state regulations.
RESPONSIBILITIES
- Completes an initial needs assessment and acts upon critical needs appropriately and immediately
- Develops a comprehensive resident-driven individual personal plan (IPP) with both short-term and long-term goals identified; creates timeline and measures for each goal
- Monitors progress towards goals in regularly scheduled interdisciplinary meetings; adjusts as needed
- Maintains resident files to include conversations, progress towards goals, and documentation of any incidents
- Assures the safety of each resident through frequent tours of the facility inspecting for any identifiable hazards
- Documents a monthly Q-note for each assigned resident
- Administers a quarterly evaluation on all assigned residents
- Implements and evaluates programs within the active treatment plan
- Maintains relations with family/guardians and case managers at healthcare facilities
- Purchase and inventory resident clothing for assigned case load
- Provides input on staff performance as requested
- Update families and guardians monthly and as needed about resident overall status
- Oversees staff adherence to scheduled case management services and activities
- Conduct employee in-service training when requested
- Acts as resident advocate during planning, development, and implementation of treatment and individual education plans (IEP)
- Serves on committees, as assigned
- Participates in quality improvement activities
- Abides by CHC policies
- Performs other duties, as assigned
QUALIFICATIONS
- Bachelor's degree in Social Work or Human Services
- Two years of supervised experience in a health care setting
- Licensed Social Worker preferred
- QIDP or QIDP eligible
- CPR / First Aid certified
WORKING CONDITIONS
- Usually works in well lighted, well ventilated area
- Requires working evenings (7:30 PM) and rotated holiday schedule
