Position Summary: Become a vital member of the NEW Home-Based Crisis Intervention (HBCI) Team in Cortland-Chenango County, NY. Make a meaningful impact on youth aged 5-20 with behavioral health needs through short-term therapeutic interventions that support families in their own homes and communities. Responsibilities:
- Provide supportive clinical services to 2-3 families at a time, conducting regular to daily meetings with each child/youth and family.
- Deliver crisis intervention, prevention, and resolution strategies.
- Engage in frequent outreach to clients and their families.
- Conduct strength-based assessments to understand clients’ social determinants of health, mental well-being, resilience to trauma, potential risks, and substance use factors.
- Create personalized service plans tailored to individual family needs.
- Design and implement effective service delivery strategies.
- Collaborate with partner service providers to coordinate comprehensive treatment plans.
- Maintain thorough case management records to ensure effective service delivery.
Education:
- Bachelors in human services, psychology, social work or other related field required
- Masters in Social Work, Psychology, Social-Welfare, Human Services or other related fields preferred
- Licensed mental health provider or unlicensed Masters preferred
Experience:
- 2 years experience in case management, community based mental health, and other related responsibilities required
- Driver's License and ability to maintain insurability throughout employment required.
