Job Location : Chisago County Government Center, Center City, MN
Position Objective
Perform comprehensive case management services to individuals eligible under Rule 79 criteria. Leverage expertise in mental health services, social services, educational services, health services, vocational services, recreational services, advocacy, transportation and more. Ability to assess and address the profound effects of housing instability, food insecurity, and other social determinants of health. This role empasizes a multidisciplinary approach, delivering person-centered, integrated care tailored to the unique needs of each individual.

- Provide on-going case management services :
Developing a functional assessment, develop and implementation of a crisis plan and individual family community support plan. Follow all federal, state and county guidelines.
Assess, reassess, and evaluate the delivery, appropriateness, and effectiveness of services over time.Do minimally one face to face visit per month as identified in statute with each identified person, telephone contact as needed.Assist the child and family in obtaining needed services by coordination with other agencies involved with the individual, child, and / or family to assure continuity of care.Assistance and outreach in obtaining a diagnostic assessment, as needed, for determination of eligibility for mental health case management and other services.Develop and implement respite care services, child foster care, psychiatric residential treatment facility or residential treatment, and aftercare services when needed. Complete out of home placement plans and other necessary paperwork for placements.Coordinate adult foster care, intensive residential treatment services, community behavioral health hospital placement, and services on the continuum of mental health care. Complete necessary paperwork.Present to the placement screening team and prepare court reports and participate in court hearings.Complete court reports and prepare for court hearings, as needed. Monitor civil commitments and / or participation in screening teams as needed.Collaborate with Child & Teen Check-up program, MN Choices Assessors and case managers, chemical health programs, child protection assessment and child welfare case management in dual case management situations.Maintain accurate and current case records as required by State, Federal and County agency regulations.Miscellaneous services
Participate in training and educational workshops.Receive other work assignments as requested.Engage in staff meetings, community meetings and regional meetings.Prepare and complete presentations as requested.Knowledge, Skills, and Abilities
Knowledge of the principles and techniques of interviewing and recording activities completed.Ability to assess and address the profound effects of housing instability, food insecurity, and other social determinants of health.Knowledge of individual and group behavior in addition to person-centered approaches.Knowledge of objectives, activities, and methods to effectively provide case management.Knowledge of socio-economic factors which promote stable family life and understanding of elements which affect family security.Knowledge of principles of community and state organization.Knowledge of physical and mental illness, impact on personality, social welfare research methods, family systems and dynamics.Ability to effectively assess, plan, refer and coordinate while monitoring and linking to services.Work constructively within the agency and utilize appropriate resources and services.Coordinate special resources and services to meet specific needs and challenges.Manage time productively, prioritize task, communicate effectively, both orally and in writing.Knowledge and adherence to HIPAA, confidentiality, and data privacy practices in the workplace.Ability to work effectively and efficiently in a variety of computer programs and systems with proficiency in keyboarding.Desired Minimum Qualifications
Bachelor's degree in one of the behavioral sciences or a related field including but not limited to, social work, psychology, or nursing from an accredited college or university.
Bachelor's degree and minimal experience, or a combination of equivalent education and experience.
