What are the responsibilities and job description for the Housing Stability Case Manager position at Clark County Homeless Coalition?
CCHC is regionally recognized leader in homeless services. We are seeking an individual to assist those experiencing homelessness with basic needs, access to shelter, and housing solutions. Duties include:
o Provide housing-focused case management to individuals and families in CCHC housing programs, or experiencing street homelessness.
o Conduct street outreach activities and connect persons experiencing homelessness to appropriate shelter and services and assist clients with daily living needs (hygiene items, clothing, doctors’ visits etc.) by providing items or services through CCHC or making appropriate referrals to resources in the community.
o Assist eligible clients to locate and obtain permanent housing solutions.
o Assist client in goal setting and assess client progress on their goals.
o Assist clients with completion of tasks relevant to: job and housing search, completion of applications, budgeting/credit management, advocacy for services, obtaining required documentation for housing applications, renter education, SSI and SSDI claims, referrals to community resources, etc.
o Facilitate and coordinate with other resources to assist clients with skills needed to achieve and maintain housing.
o Maintain detailed client files with all required documentation in a timely manner.
o Represent CCHC in community outreach events and meetings.
o Travel is required to meet with clients, community partners, attend outreach events, community meetings, workshops, trainings, etc.
Qualifications:
o BSW or degree in human services field required with a minimum of two years’ experience.
o Strong written, verbal and organizational skills.
o Excellent computer skills, especially Word, Excel, and email; ability to use basic office machines-copier, fax, etc.
o Valid Driver’s License and valid insurance.
o Must be able to climb stairs and lift at least 25 pounds.
o Self -directed; able to follow through assigned tasks to completion with minimum supervision, excellent problem-solving skills, pleasant personality, and cooperative spirit.
Job Type: Full-time
Pay: $35,000.00 - $40,000.00 per year
