The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
Under general supervision, the Health Outreach Program (HOP) Coordinator is responsible for safely and efficiently driving the mobile medical unit (40ft RV) between its home location and various destinations within the metro Denver area. The HOP Coordinator ensures the proper setup and breakdown of the unit at service locations, maintains the vehicle's operating condition and cleanliness, and oversees the completion of regular maintenance tasks (weekly, monthly, and annual). Additionally, the Coordinator serves as the liaison between the Stout Street Health Center and the HOP sites. This position also supports the health team by managing client flow, including handling client registration, appointment check-ins, and other tasks within the Electronic Health Record (EHR) system.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
- Oversee the general operations of the HOP program, including driving a 40ft RV, scheduling, site coordination, and maintaining positive community and provider relationships.
- Safely drive the HOP mobile medical unit to service locations and back, ensuring timely and safe transport.
- Ensure the mobile medical unit is properly maintained by adhering to its scheduled maintenance and keeping accurate records of maintenance tasks.
- Actively participate in identifying homeless and at-risk individuals and families, helping them access health and community resources.
- Support patients by assisting them in scheduling with other services offered at Stout Street Health Center that are not accessible on the HOP.
- Communicate with clients, providers, and the broader community in a professional and empathetic manner.
- Maintain and update the HOP training manual and train staff on mobile unit operations and safe driving practices.
- Order and maintain medical supplies, pharmaceuticals, and other necessary stock items for the mobile unit.
- Maintain a professional and confidential work environment in all interactions.
- Ensure adherence to safety standards and maintain complete COVID-19 vaccinations, as defined by the CDC and local health authorities.
- Providing safe delivery of patient care in routine and emergency situations, within the scope of education and training, such as taking vital signs and performing patient assessments.
- Performing all medical assistant requirements when needed in the clinic, including:
- Checking in adult and pediatric patients.
- Taking vital signs appropriately.
- Administering immunizations and recording them into the CIIS and/or NextGen.
- Performing venipuncture, obtaining specimens, and conducting all laboratory tests.
- Conducting basic medical office procedures, such as using pulse oximetry, peak flow meters, EKG, and other screening tests.
- Assisting the Behavioral Health Professional (BHP) with patient check-ins and care of regularly scheduled patients.
- Recording all vital signs into NextGen, including BP, P, T, R, Height, Weight, smoking status, and glucose levels or peak flow if applicable to the patient’s diagnosis.
- Recording all patient visits electronically into NextGen, ensuring the inclusion of PMH, PSH, FH, Social History, and allergies.
- Entering immunizations into NextGen.
- Performing other assigned duties and responsibilities.
- Other Duties:
- o Perform other duties as assigned to support clinic operations and organizational needs.
- High school diploma or equivalent.
- Certification in basic life support (CPR).
- EMT or Medical Assistant (MA) Certification
- Bilingual (English/Spanish).
- Large RV or vehicle driving experience.
- Previously worked with Non-Profit organizations.
