Summary:
The Referrals/Medical Records Clerk works as part of the medical office team performing the day to day tasks to ensure referrals and medical records are documented and processed in a timely and accurate manner. ONSITE POSITION.
Responsibilities:
· Ensure confidentiality.
· Ensure compliance with HIPAA.
· Stay up to date on legal requirements for medical records processing.
· Process records requests and referrals in a timely and accurate manner.
· Processing prior authorizations for referrals.
· Tracking the process and completion of referrals and updating patient and provider on status of referral i.e. delay, insurance problems etc.
· Processing incoming and outgoing release of information in a timely manner.
· Working with other facilities to coordinate appointments for patients.
· Assists patients with overcoming barriers to care.
· Faxing and/or mailing patient information to other facilities.
· Keep all medical records secure according to CRH standards.
- Scan and index all lab, EKG, X-ray, and other patient communications and reports in appropriate areas of patient’s electronic chart.
- Distribution, pick up, and scanning of paperwork that requires action.
- Retrieve needed patient records for patient care.
· Work cooperatively and supportively with all CRH staff, patients, and the public.
· Assists with coverage in other Revenue Cycle Depts as needed.
· Other duties as assigned by Front Office Manager and Revenue Cycle Director.
Physical Demands and Work Environment: Work is primarily in an office environment but may occasionally require working outdoors for projects and/or events. May include travel for patient care, training, events. May involve bending, hearing voice conversations, keyboarding, kneeling, lifting up to 35 pounds, pulling, pushing, reaching, sitting, standing, walking, reading, writing, and proofreading. Employee shares common exposure to communicable disease.
Reporting Relationships: The Referrals/Medical Records Clerk reports to, and is supervised and evaluated by, the Front Office Manager.
Evaluation: The evaluation of work performance is ongoing and will be performed by the Front Office Manager. It will include the specific duties and responsibilities of this position’s description plus employee attitude, general working behavior and attendance. Formal evaluation will normally be performed annually but may be instituted at other times by the employee or supervisor.
Qualifications/Education & Experience:
· Previous experience in Referrals and/or Medical Records preferred. (Applicant with at least 6 months of experience in medical field may also be considered.)
· High School graduate or GED equivalent.
· Basic Life Support certification or ability to complete within 6 months of employment.
· Must be a person of high integrity.

· Bilingual English/Spanish preferred (not required).
· Previous experience in a health care setting preferred, ideally in an office practice.
· Must be able to complete Basic Life Support training class.
· The ability to carry out the assigned duties of this position independently or with only minimal guidance and/or supervision.
· The ability to relate effectively to providers, patients, representative of third-party payers, and other groups/clinics/providers.
