The Continuing Care Community Health Worker is responsible for working in conjunction with an interdisciplinary team in order to improve patient social, behavioral, and mental health needs through outreach and engagement activities. Such activities include telephonic and written communication, as well as in-person opportunities with patients, providers, team members, and community-based organizations.
Work may be focused on, but not limited to: 1) establishing effective relationships and guiding the patient/caregiver through the healthcare system, working to eliminate barriers that might otherwise adversely impact patient care/outcomes; 2) providing general education/training designed to promote self-awareness and reinforce/maximize patient/caregiver self-management skills/tools/resources; and 3) facilitating access to support services, community resources and primary care for enrolled patients and tools for self-management support.

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
This full time remote role works Monday through Friday 8:00 am to 4:30pm (mountain time). We are looking for candidates that have at least 1 year of experience in a patient care environment or 2 years working ain a health related community outreach organizations. We are looking for candidates that have experience with electronic medical records and healthcare terminology.

Qualifications

- High School Diploma/G.E.D.
- 1 year experience in a patient-care environment; or 2 years working in a health-related community outreach organization.
- Preferred Education: AA or BA.
- Preferred Experience: Knowledge and ability to work with electronic medical records; knowledge of healthcare terminology.
- Preferred: CHW Certification
- Excellent customer service skills.
- Ability to collaborate with interdisciplinary and community partners for a mix of clinical, operational, and business activities related to that partnership.
- Ability to communicate effectively using written and verbal skills. Includes strong listening skills, listening for understanding.
- Excellent computer literacy, including ability to learn new systems. Proficient in email communications and internet usage along with basic use of electronic software for applications including documents, spreadsheets and slide presentation (google product line).
- Able to adapt to changing environments and priorities.
- Understanding of and sensitivity to mental health conditions and addictive disorders.
- Ability to demonstrate skillful decision making and judgment including time management.
- Knowledge of cultural diversity/issues/barriers and low literacy issues.
- Ability to assist in the development and implementation of effective plans for the continuing care needs of patients.
