DESCRIPTION OF WORK
With our team, you will assist those who are returning to the community after mental health hospitalization or preventing premature hospitalization. This work supports the consumer's efforts to reintegrate into or remain in their community, thus fostering their success in recovery and in building the confidence and self-reliance needed for full independent living.
This position supports each consumer's efforts consistent with their individual goals, plans, and objectives. You will train consumers in home safety protocols, share observations with your supervisor and team members as appropriate, and document your observations of consumers' behaviors and progress in a timely manner. You may also be expected to transport consumers in a county vehicle to appointments, when necessary.
This position requires close contact with our consumers in their home environment. Workers in this capacity will work on shifts to assure 24-hour availability in a small facility in which a limited number of seriously mentally ill consumers reside, each in their own apartment. Work Schedule and Additional Information :
- Part-time employment, approximately 15-20 hours per week.
- Work hours are Tuesday, Thursday, and Saturday. Employee may have occasional overnight travel or out of County travel for meetings or training and often works irregular hours, including evenings, holidays, and weekends may be required.
