Job Overview:
We are seeking a dedicated and compassionate Lead CFSS Consultation Case Manager to join our team. This leadership role is essential in coordinating client care and overseeing service planning to ensure individuals receive the support they need, tailored to their unique goals and circumstances. In addition to direct case management, this position will be trained in MnCHOICES assessments and Service Agreement processes, helping guide our team through service eligibility and planning. The ideal candidate will demonstrate strong leadership skills, clinical insight, and a commitment to equity and person-centered care.
Key Responsibilities:
- Serve as the lead point of contact for CFSS consultations and support coordination.
- Conduct comprehensive assessments of clients’ needs, including medical, social, and functional factors.
- Develop and oversee individualized care and service plans that align with each person’s preferences and goals.
- Coordinate services across disciplines and community providers to ensure continuity of care.
- Supervise or mentor other case managers, offering guidance and support as needed.
- Maintain accurate, up-to-date client records while strictly adhering to confidentiality standards.
- Complete and manage MnCHOICES assessments and assist in creating and monitoring Service Agreements for eligible individuals.
- Provide education to clients and families about available services, eligibility, and ongoing support options.
- Monitor client progress and revise service plans in response to changing needs.
- Advocate for clients within the service system, addressing barriers to care and access.
- Stay informed about changes in DHS policies, CFSS guidelines, and best practices in case management.
Qualifications:
- Bachelor’s degree in Social Work, Public Health, Human Services, or related field required (Master’s preferred).
- Previous experience in case management, particularly in medical, disability, or community-based settings.
- Strong understanding of medical, behavioral, and social service systems.
- Experience or willingness to be trained in MnCHOICES and Service Agreement processes.
- Excellent communication and interpersonal skills, with the ability to support and lead a team.
- Strong organizational skills with the ability to manage multiple priorities.
- Commitment to person-centered, culturally responsive, and trauma-informed care.
