What are the responsibilities and job description for the Social Worker position at Community LIFE?
Summary:
Our Social Workers maintain a caseload of Community LIFE participants, adults over 55 years old, providing psycho-social assessments and interventions for their well-being in the community. As the primary contact, they perform utilization case management for acute inpatient admissions and transfers.
This is a Monday-Friday position from 7:30am to 4:00pm without nights or weekends.

Requirements
To be successful in this role, you will need:
- A Master's Degree in Social Work (MSW) from an accredited school
- A minimum of one year's experience working with the frail or elderly population
- Geriatric social work experience in a hospital, long-term care, or community setting
You should possess:
- Good verbal and written communication skills
- The ability to work with people from various socio-cultural backgrounds
- The ability to accept and utilize supervision
- Self-direction and good problem-solving skills
- A compassionate and empathetic attitude towards older adults
In addition, you must have:
- A valid PA driver's license
