The Social Worker's role is to respond to each resident's social needs from the time of admission and throughout the resident's stay, collaborating with other staff and professionals both in and outside the facility in developing and implementing coordinated individualized plans of care and supportive services. Social Workers must embrace the person-centered care approach of Presbyterian SeniorCare Network based upon person-directed values and practices where the voices of elders and those working and living with them are considered and respected. We need a Social Worker who will help our residents by supporting their daily living needs and develop trusting, caring relationships. Our Social Workers are required to :
A degree in Social Work, Gerontology or other related field is preferred.
