Children's Hospital of Pittsburgh is hiring a full time Social Worker to work at the main hospital location in Lawrenceville.
Responsibilities include conducting psychosocial assessments, crisis intervention, collaboration and program development with organizations such as schools, employers and community groups, support for guardianship and disability process, and assistance with transition to adult care.
- Provide resource-specific information to patients and families, initiate contact with appropriate resources, and facilitate referrals.
- Mentor social work students and serve as a field instructor to MSW students.
- Utilize therapeutic counseling methods such as crisis intervention, psychosocial support, brief goal-directed therapy, and CISM.
- Document social work interventions accurately and timely in the medical record.
- Educate patients and families on reimbursement systems and advocate for continued care authorizations.
- Identify, assess, counsel, and intervene with medically and psychosocially complex patients.
- Monitor and evaluate the plan of care throughout the hospital stay, reassessing every 3 days or as status changes.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Educate and advocate for patients and families regarding medical care, discharge planning, advance directives, and end-of-life care.
- Complete psychosocial assessments considering social, economic, cultural, age-related, and behavioral factors.
