Description
Verland is now hiring a full time Social Worker to join us at our Main Campus in Sewickley, Pa. This is a salaried, exempt position.
Contributions
- Interacts with individuals, family members, guardians, staff, or community agency staff to plan, provide, or coordinate needed social work services.
- Serves as the coordinator in the ICF/IDD or ICF/ORC for new referrals, admissions, discharges, or transfers.
- Provide tour of the ICF houses where vacancies are to interested families.
- Gather and document information into the EHR for Inquiry/Referrals.
- Serves as the Chair for the Admissions/Discharges Committee.
- Works with single county authorities to ensure appropriate referrals have on-site tours and off-site observations are conducted by Team as a part of the acceptance or denial process.
- Collects insurance information to assure it is current and shares copies of insurance information with the Nursing and Finance Department by Admission.
- Participates in the development of the client’s comprehensive care, treatment, or support service plans, including Social Work Evaluations, Discharge Plans, annual reports, and quarterly reports. Documents all interactions with clients/families/legal guardians in the electronic health record.
- Interviews clients, their family members, legal guardians, and external agency representatives to obtain, update, and record information regarding their physical, mental, emotional, economic, and social status.
- Participates in multidisciplinary treatment team meetings to discuss psychosocial assessments and family histories.
- Ensures needed benefits and services are obtained for clients in accordance with their treatment plans and applicable mandates, regulations, and policies.
- Identifies and addresses potential family dynamics that may impact individual care.
- Discusses the Legal Guardian process and emails information to the participant about Legal representation at a possible reduced rate.
- Interprets federal, state, departmental and facility social work legislation, regulations, policies, and procedures.
- Responds to correspondence related to court documents, parental or relative support, rate payments for care and maintenance, and public and private insurance reimbursements.
- Acts as liaison with school systems to ensure resident service delivery and continuity of care.
- Provides emotional support to individuals, families and staff as needed. (Counseling is done by the Behavioral health dept.) This SW is not involved in counseling, only support, guidance, etc.
- Gather and report all “Leave of Absence” correspondence.
- Collaborate with the IDT Team to ensure a sharing of information to track Community Outings, Visits, etc.
- Collaborate with the CLA Leadership Team to ensure communication about inquiry/Referrals for their program.
- Collaborate with the EHR Team to ensure all fields and screens for tracking the Inquiry/Referrals are completed.
- Communicate daily or as scheduled, with the SW supervisor as scheduled. Include the supervisor in all new developments and changes to the Social Work role as assigned by others.
- Serves on the Human Rights Committee and will review/provide in-services to staff as needed.
- Performs all other related duties as assigned and reacts positively to change.

- Bachelor’s degree in social work is required.
- Master’s degree in social work is preferred.
- Active Social Worker license in the state of PA; in good standing.
- Two years of professional experience providing direct public or private social work services.
- Act 34 Child Clearance with Renewal, Act 34 Criminal Clearance with Renewal, and Act 73 FBI Clearance with Renewal.
- Must have own reliable transportation, good driving record, and car insurance.
