Key Responsibilities
- Collaborate with clients and the treatment team to develop, implement, and refine personalized treatment goals and plans.
- Manage and oversee the medication administration system, ensuring it aligns with individual treatment plans and is delivered effectively.
- Provide education and training regarding medications and mental health to both clients and team members.
- Conduct comprehensive evaluations of clients’ psychiatric histories, mental statuses, and overall health needs.
- Attend daily organizational meetings and weekly supervisory sessions to ensure cohesive team dynamics.
- Complete necessary documentation in accordance with agency policies, including assessments and treatment plans.
- Offer on-call crisis intervention during evenings, weekends, and nighttime, ensuring continuous support for clients.
- Accompany clients to medical appointments when needed, facilitating communication regarding health issues and treatment.
- Collaborate with the ACT team on health and medication-related concerns for clients.
- Strong clinical assessment capabilities.
