Job Overview

We are actively seeking a dedicated Nurse (RN) to join our Assertive Community Treatment (ACT) Team, where your expertise will directly contribute to the well-being of individuals facing chronic mental health challenges. Key Responsibilities

• Collaborate with clients and the treatment team to develop, implement, and refine personalized treatment goals and plans.
• Manage and oversee the medication administration system, ensuring it aligns with individual treatment plans and is delivered effectively.
• Provide education and training regarding medications and mental health to both clients and team members.
• Conduct comprehensive evaluations of clients’ psychiatric histories, mental statuses, and overall health needs.
• Attend daily organizational meetings and weekly supervisory sessions to ensure cohesive team dynamics.
• Complete necessary documentation in accordance with agency policies, including assessments and treatment plans.
• Offer on-call crisis intervention during evenings, weekends, and nighttime, ensuring continuous support for clients.
• Accompany clients to medical appointments when needed, facilitating communication regarding health issues and treatment.
• Collaborate with the ACT team on health and medication-related concerns for clients.

Qualifications

• An Associate’s or Bachelor’s degree in nursing is required.
• Prior experience in an inpatient or outpatient psychiatric setting is essential.
