RN Nurse for Assertive Community Treatment (ACT)

Job Overview

Compass Health Network is seeking a dedicated and compassionate Nurse (RN) to join our Assertive Community Treatment (ACT) Team. This full-time position, essential to addressing the critical healthcare needs of our communities, offers a $4,000 sign-on bonus.* As a member of our ACT Team, you will play a vital role in delivering high-quality psychiatric care while collaborating closely with clients and a multidisciplinary team.

Key Responsibilities

• Conduct thorough psychiatric assessments and evaluate physical health needs.
• Collaborate with clients and treatment teams to create, implement, and adjust individualized treatment plans.
• Administer and manage medication in conjunction with the psychiatrist, ensuring integration with treatment goals.
• Provide education and support to clients and their families about medications and mental health conditions.
• Participate in comprehensive assessments covering mental health history, physical health, and daily functioning.
• Attend and contribute to daily staff meetings and treatment planning discussions.
• Maintain accurate documentation in compliance with agency policy, including progress notes and treatment plans.
• Offer on-call crisis intervention and accompany clients to medical appointments as needed.

Qualifications

• Associate’s or Bachelor’s degree in nursing is required.
• Experience in a psychiatric setting, either inpatient or outpatient, is essential.
