Job Summary
This is a full time (33.5 hours/week) position.
The Homebased Crisis Intervention Interventionist provides intensive services to children and their families, identified as at imminent risk for inpatient psychiatric hospitalization. Reports directly to the supervisor.
Responsibilities and Duties
- DAILY
- Provides intensive (6-10 hours per week) short-term crisis-oriented intervention to up to three families concurrently for a maximum of eight weeks.
- Responds to new referrals as soon as possible, but no later than 24 hours of referral to the program.
- Conducts a complete lethality assessment of new clients and develops a safety plan when appropriate.
- Assists families in identifying factors/life stressors leading to the crisis, creating the risk of hospitalization.
- Collaborates with the client’s community based mental health professionals in order to supplement ongoing treatment or to assist the client in obtaining community based mental health services to reinforce the gains made during the intensive intervention and maintain stabilization.
- Utilizing the Homebuilders® Model, teaches skills to families which will lessen the possibility of psychiatric hospitalization.
- PERIODIC
- Follow up with families to both assess their progress and to evaluate the program’s success.
- Keep comprehensive client records including HBCI forms, OMH required forms as well as treatment plans, weekly summaries and termination letters.
- Participates in weekly group case consultation with HBCI Team and Supervisor.
- AS REQUIRED
- Attends training as suggested by supervisor and/or identified opportunities for professional development.
- Participates in quality assurance activities for the HBCI Program.
- Other duties as assigned.
Qualifications and Skills
QUALIFICATIONS:
A MSW or equivalent required, license preferred. Home visiting experience is preferred. An awareness of community resources and an understanding and positive regard for the needs and interests of families served is a must. Experience developing, implementing, and evaluating behavioral plans is preferred. Salary is dependent on degree and experience.
This person must work well with staff and other community providers; possess excellent written and oral communication skills. A valid Driver’s License, and reliable transportation is required. This is a full-time position.
