Position Title: Case Manager

Status: Hourly – Part time - 20 hours per week, flex scheduling based on client needs

Compensation: $18.00 per hour

Required: Valid Driver’s License, clean driving record, personal reliable transportation, personal vehicle insurance, ability to pass MDHHS background check, criminal record check, FBI fingerprinting, and TB test

Bachelor’s degree or working towards a degree in a social services field

Lived experience in homelessness, foster care, juvenile justice, or as a member of a subpopulation


Staff in this position work with young adults and/or pregnant/parenting youth 18 to 24 years old who are experiencing homelessness. The case manager will work with community partners, HARA, McKinney-Vento, and Foster-care youth liaisons to assist youth in securing housing and support services. Staff are required to provide care in a positive youth development and trauma informed care approach, with the goal of assisting youth to achieve self-sufficiency. Case managers are assigned a designated county in the Thumb region of Michigan with an active, revolving caseload of up to seven youths. Counties include Saint Clair, Sanilac, and Huron, it is preferred that the case manager reside in the county they are assigned to. 

 

Position Duties: 

  • Participate in community, partnership, and agency meetings regarding housing coordination, continuum of care (CoC), local planning body (LPB), YHDP, supervision, case consultations, and training. 
  • Complete intakes, assessments, case management, discharges, and required client file documentation in a timely, accurate, and legible manner.
  • Use motivational interviewing to assist youth in determining the best housing path for themselves.
  • Complete HMIS intake, self-sufficiency matrix, exit, follow-up, and annual assessments with each youth.
  • Implement an individual service plan with each youth to:
    • Build a foundation of life skills such as financial literacy, career building, education opportunities, self-advocacy, household management, and parenting when appropriate.
    • Develop and monitor youth progress towards self-identified goals in the service plan.
    • Assist youth in securing safe housing, basic needs, and referrals for supportive services as requested.
  • Provide youths with referrals to and attend appointments with community partners
  • Monitor success of referrals provided and outcomes to ensure youths needs are met.
  • Assist youth with applications for education, employment, services, and personal identification needs.
  • Schedule home case management visits with each youth weekly.
  • Monitor and support the youth’s compliance with the landlord/tenant agreement.
  • Provide food, clothing, shelter, basic needs, medical assistance, advocacy, and other needs that arise.
  • Track receipts in youth files, track billing and submit to the financial department expeditiously.
  • Coordinate HUD required INSPIRE housing inspections for compliance with safety prior to move-in.

Organizational Duties: 

  • Complete training as required by the agency, program demands, funding streams, and individual development.
  • Compile data and narrative descriptions for quarterly, semi-annual, and annual reports.
  • Participate in public relations activities, outreaches, community education, and presentations.
  • Ensure policies, procedures, and federal, state, local, and tribal laws and regulations are followed.
  • Complete and submit accurate and timely payments and invoices for rental assistance and support services.
  • Track client service hours and submit monthly reports on the last day of the month. 
  • Perform all other duties as assigned by supervising staff.

Organizational Responsibilities to Staff: It is expected that the program manager will train, support, and mentor new staff members. All work materials are provided. Clinical, case management and HR support is available to all staff.
