Position Summary
The Licensed Practical Nurse Assessment Coordinator (LNAC) is an LPN who is responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and medical records for residents.
Qualifications:
Licensed Practical Nurse (LPN) currently licensed by the state of Pennsylvania
Two (2) years nursing experience preferred but not required
Six (6) months of Geriatric nursing experience preferred but not required
Must be self-motivated and willing to engage in not only hands-on care, but to follow-up with documentation compliance.
Must possess CPR and First Aid certificates or willing to obtain each within six (6) months of employment.

Essential Duties and Responsibilities
1. Coordinates, conducts, and completes the MDS, family/resident care plans and conferences on the computerized system. This includes documenting all services provided in order to assure accurate reimbursement from all payer sources and assuring/monitoring that MDSs are completed within HCFA and Medicare guidelines.
2. Assures accuracy and completeness of entire medical record. This includes assuring the accuracy of the written care plan initiated by the nursing supervisors upon admission and attending care conferences.
3. Accurately assesses resident needs making sure that all residents’ care is being provided per established care plans and that ADL documentation is correct.
4. Assures effective communication with other shifts and departments as it relates to medical record documentation.
5. Works with managed care companies to provide clinical updates and reviews. Keeps current on managed care updates.
6. Assists with utilization reviews of clinical management including assisting with discharge planning. Keeps current on the financial aspects of nursing operations.
7. Works on-call to assist nursing staff with resident care.
8. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
