PCM & Consulting LLC is seeking an experienced Case Manager to join our team. This full-time, Monday-Friday position focuses on providing hands-on case management for auto-injured individuals, workers’ compensation cases, and geriatric clients. The ideal candidate will have a background in nursing, social work, or case management and a passion for helping clients navigate their recovery journey.

Key Responsibilities:
- Assess and analyze medical files to evaluate treatment progress.
- Develop and implement individualized care plans to support client recovery.
- Coordinate care by working closely with healthcare providers, injured clients, and other stakeholders.
- Accompany clients to medical appointments, ensuring they receive appropriate care and advocating for their needs.
- Maintain detailed case notes and reports to track client progress and support decision-making.
- Advocate for maximum medical improvement, ensuring effective, high-quality, and cost-conscious care.
- Work with auto-injured individuals, workers’ compensation cases, and geriatric clients to facilitate recovery and long-term well-being.
Qualifications & Requirements:
- Valid driver's license and auto insurance (required for field-based work)
- Active licensure as one of the following:
- Registered Nurse (RN)
- Licensed Practical Nurse (LPN)
- Social Worker (LMSW, LBSW, or equivalent)
- Medical Case Manager
- Care Manager
- Minimum of two years of case management, healthcare coordination, or related experience.
- Strong knowledge of auto injury case management, workers’ compensation cases, and geriatric care.
- Valid driver’s license and auto insurance (required for field-based work).
- Excellent organizational, communication, and problem-solving skills.

- Ability to Commute: Within Wayne, Oakland, Macomb and nearby counties
- Work location: Royal Oak, MI 48067 and / In the field/ within the community
