Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. This role involves assessing clients' needs, developing treatment plans, and facilitating access to resources that promote well-being. The Social Worker will work collaboratively with the interdisciplinary tream, other healthcare providers, community organizations, and families to ensure comprehensive care.
Duties
- Conduct assessments of clients' needs and develop individualized treatment plans.
- Provide counseling and support to clients dealing with various issues, including mental health and family dynamics.
- Maintain accurate medical documentation and manage medical records in compliance with regulations.
- Engage in case management by coordinating services and resources for clients.
- Advocate for residents and provide support tailored to their unique needs.
- Collaborate with community resources and agencies to facilitate care upon discharge.
Skills
- Proficiency in medical documentation and coding practices.
- Strong case management skills with the ability to coordinate multiple services effectively.
- Experience in long term care or related fields is a plus.
- Knowledge of motivational interviewing techniques to facilitate client engagement.
- Excellent communication skills, both verbal and written, for effective client interaction.
