General Functions: Serve as an integral member of the behavioral health team at a large social services agency. Primary functions include coordinating the screening and admission process, conducting intakes, completing mental health assessments, and leading the treatment planning process for adults with mental health and co-occurring substance use disorders.

- Respond to referral calls and provide appropriate external referrals, as needed.
- Manage the waiting lists for various programs.
- Review referral packets.
- Conduct screening and intake interviews.
- Schedule and coordinate needed assessments.
- Complete initial and semi-annual clinical assessments for individuals served according to agency policies and procedures.
- Develop, coordinate, and document treatment planning in accordance with established documents, timeframes, and criteria.
- Participate in meetings and trainings for various programs to support outpatient and supportive housing projects.
- Using a team approach, communicate the results of the screening and intake process with supervisor and the appropriate Coordinator and Associate Director.
- Consult with team members and family/guardian of individuals served when appropriate to coordinate care efforts, share individual needs, and discuss assessment results.
- Report observations, findings, and needs to therapists and appropriate case manager to ensure staff is aware of services needed by individual served.
- Deliver and document mental health services to meet the needs of persons served as indicated in their individual treatment and recovery plans.
- Record the process and results of services within established timeframes and in sufficient detail to meet Medicaid Rules 132, 140, and/or agency and other insurance billing requirements.
- Participate in monthly clinical supervision meetings, seek ongoing clinical supervision from supervisors, and participate in continuing education to maintain applicable licenses/certifications.
- Serve as a liaison to other community agencies, ensuring that staff are informed of available services and resources in the community.
- Work in conjunction with Community Housing Coordinators and the Manager of Properties and Landlord Relations to assure smooth transition into residential programs.
- Facilitate agency trainings and participate in agency committees as appropriate and assigned.
- Obtain and maintain annual IM CANS certification per Medicaid Rule 140.
- Meet or exceed assigned weekly billing quotas as determined by the Director of Professional Services.
- Be on-call for telephonic-only crisis intervention services in accordance with the staff rotation on the weekly crisis line schedule. Crisis phone provided.
- Participate in all agency and department trainings, meetings, online trainings, and any additionally recommended trainings or meetings as required.
- Maintain compliance with all certification requirements, such as First Aid/CPR, Safety Care, etc.
- Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
- Other duties as assigned by the Manager of Intake and Clinical Services, Coordinator of Community Housing, Associate Director of Professional Services, and Director of Professional Services.
- Operate agency or personal vehicle in accordance with established policies and procedures.
- Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
- Maintain ongoing compliance with all agency-related training and certification requirements.
- Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
- Operate agency or personal vehicle in accordance with established policies and procedures.
- Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.

Qualifications: Must meet QMHP requirements per Medicaid Rule 140: Master's degree in counseling, social work, counseling/clinical psychology, or related field required with successful completion of 1,000 hours of practicum and/or internship under clinical and educational supervision; or one year of documented clinical experience under the supervision of a QMHP. Proficiency in the use of computers and typing, and working knowledge of MS Word, MS Excel, and MS Outlook applications required. Bilingual a plus. Driving record and auto insurance coverage in compliance with agency policy. Post offer physical and criminal background check required. Reliable transportation required. EOE.
