POSITION SUMMARY:
Under the direction of the Director of Social Services, the CMS is responsible for the daily oversight and performance of eight case managers in a men emergency shelter. The position will ensure the delivery of quality services to all clients on a 24-hour basis as per CPU and NYC Department of Homeless Services (DHS).
The Case Work Supervisor will work closely with Director of Social Services to provide support co Case Managers in coordinating and delivering high quality of care to people served.
POSITION REQUIREMENTS, EDUCATION & SKILLS:
• Assign cases to Case Managers and overseeing their work to ensure that they are meeting the standards of performance correctly and efficiently
· Ensure that ILP are conducted bi-weekly and that clients are seen on a weekly basis
· Provide staff training on areas of their work necessary for good delivery of services
· Arrange for case presentations
· Hold morning huddles with case managers to review and plan for corrective plan of action
· Schedule work according to priorities
· Hold individual and group supervision with Case Manager’s team
· Provide individual counseling, advocacy and support to clients when necessary to review cases and address problems.
· Maintain necessary records and prepares regular and special reports as required∙
· Engage in community outreach efforts to identify housing opportunities.
· Develop and maintain relationships with government partners, and supportive community-based organizations.
· Coordinating services with other agencies to ensure that clients receive the care they need. * * *Serve as a resource to the shelters’ social services department staff.
· Represent CPU at city and statewide coalitions, task forces and committees as necessary.
· Participate in monthly staff meetings, case conferences and training seminars as indicated by Director of Social Services Assist with reports and program evaluations.
· Perform other duties as assigned by the Director of Social Services.
· Interprets regulations, policy and changes for the Case Managers
· Able to flex schedule when necessary
KNOWLEDGE AND, SKILLS
· Bachelor’s degree with 3 or more years of experience working with disenfranchised communities including individuals who identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, and individuals with mental health issues and the homeless population
· Master’s degree in Human Services of related field plus one year of experience working with vulnerable population.
· Additionally, possess knowledge and skills necessary to develop strong linkages with city and community-based services in order to secure services timely for clients.
· Must have the ability to be flexible with schedule as needed
· Must be computer-literate and must have knowledge of Microsoft Office applications.
· Strong computer skills including Microsoft Word, excel and Power Point
· Knowledge of CARES DHS System preferred
