Primary Responsibilities
The Director of Palliative Care is responsible for the day-to-day successful operations of the Palliative Care program consistent with the mission, vision, and values of Crater Community Hospice (CCH). The Director of Palliative Care is a dynamic leader with a proven history of leading a team of clinicians in supporting the continuum of care that has been established with attention to customer satisfaction, fiscal responsibility and quality control.
Essential Functions
- Provides inpatient and outpatient consultation services in palliative care, including symptom management and clarification of patient and family goals of care.
- Consults with the primary provider and the Interdisciplinary team to establish and document a written focus of care specified in the treatment plan.
- Facilitates access to appropriate supportive care services and venues.
- Provides ongoing education to palliative team and allied professionals/trainees on fundamentals of symptom management and to support goals of care conversations for care transition.
- Participates in weekly interdisciplinary group to review active issues of hospitalized and home-based patients under palliative care management.
- Builds effective relationships with referring physicians, identifies opportunities to improve patient care, and improve efficiency and effectiveness of the program.
- Collaborates with others as appropriate to ensure that palliative clinical protocols, utilization review, case management, quality assurance, and compliance programs are implemented according to CCH policies while ensuring compliance with legal, regulatory, and clinical policies and procedures.
- Develops a cooperative relationship and communicates effectively and professionally with direct report nurse practitioners.
- Investigates and reports any problem relating to patient care or conditions which might harm the patient and/or an employee’s well-being.
- Attends pertinent continuing education programs other than routine in-services and shares information with staff.
- Works with the CEO in identifying budgetary requirements and determining appropriate use of allocations.
- Consults with leadership to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of health personnel.
Knowledge, Skills, and Education Requirements
· Current RN, APRN license by the State Board of Medicine to practice in Virginia, with at least three (3) years of clinical experience beyond state licensure, and experience in Palliative Care preferred.
· Board certified or eligible in Hospice/Palliative Medicine or equivalent experience.
· At least one year of experience as a supervisor and working as lead of interdisciplinary team.
· Effective interaction with all team members, senior management, and Board of Directors, with a commitment to provide exceptional communication through excellent interpersonal and coaching skills with a friendly and positive attitude.
· Ability to work effectively in a fast-paced environment; effective time and project management, flexibility, problem solving, and strong organizational skills.
· Strong commitment to palliative philosophy and familiarity with non-profits and the medical industry is strongly preferred.
Work Environment & Physical Demands
Situated in a professional office environment, this role is largely sedentary with rare lifting and/or carrying up to 20 lbs. This role routinely requires the use of standard office equipment such as computers, phones, copiers, filing cabinets and fax machines. Occasional local travel as required for professional meetings and patient care.

Pay: $90,000.00 - $120,000.00 per year
