Position Overview : As the Multi-Site Program Coordinator, you will play a pivotal role in overseeing and enhancing our ICS housing programs. This leadership position requires a visionary individual with a deep understanding of 245D Compliance regulations, proven program management skills, and a commitment to delivering high-quality services. The successful candidate will lead a dedicated team in providing integrated community support services, with a focus on individuals with disabilities, mental health issues, behavioral, chemical dependency, and crisis management. Additionally, the role involves actively engaging with support staff and clients.
Key Responsibilities :
Develop and implement program goals, objectives, and strategies aligned with the organization's mission and vision.
Oversee the day-to-day operations of the multi-site programs, ensuring efficient and effective service delivery.Manage program budgets, including forecasting, monitoring expenses, and adjust as needed.Provide ongoing coaching and development of all employees.Supervise and support a team of Integrated Community Support Professionals, providing guidance, support, and professional development opportunities.Foster a positive and inclusive program environment that promotes teamwork and collaboration.Provide supervision, support, and evaluation of activities that include : (1) oversight of the license holders responsibilities assigned in the persons support plan and the support plan addendum; (2) taking the action necessary to facilitate the accomplishments of the outcomes according to the requirements in section 245D.07; (3) instruction and assistance to support staff implementing the support plan and the service outcomes, including direct observation of service delivery sufficient to assess staff competency. The multi-site program coordinator may delegate the direct observation and competency assessment of the service of delivery activities to support staff to an individual whom the multi-site program coordinator has previously deemed competent in those activities; and (4) evaluation of the effectiveness of service delivery, methodologies, and progress on the persons outcomes based on the measurable and observable criteria for identifying when the desired outcome has been achieved according to the requirements in section 245D.07.Performance of other duties as assigned.Client Support and Advocacy :Ensure the delivery of high-quality integrated community support services with a person-centered and inclusive approach.
Conduct client assessments and support planning processes, ensuring compliance with relevant regulations and policies.Collaborate with case managers, support staff, and persons served for joint problem-solving.Foster positive outcomes through personalized goal setting, skill building, and community resource connections.Develop, maintain, and review ISPs, IAPPs, SPAs, and SMAs.Team Management and Development :Conduct regular team meetings to discuss progress, challenges, and professional development.
Oversee training for employees in all areas of program implementation.Provide guidance, support, and supervision to the team, fostering a supportive work environment.Collaborate with other departments to integrate services and address clients' broader needs.Develop and implement employee schedules.Complete employee evaluations, conduct investigations, and initiate disciplinary actions and terminations as needed.Evaluation and Improvement :Monitor and evaluate program outcomes, initiating improvements for service quality and client satisfaction.
Utilize data to identify trends, assess program effectiveness, and make informed decisions.Conduct periodic reviews of client files and documentation to maintain compliance.Evaluate program effectiveness and identify areas for improvement, implementing changes as needed.Qualifications :
Strong administrative skills, including budgeting, program development, and program management.Strong understanding of 245D licensing compliance.Excellent leadership and interpersonal skills with the ability to build and maintain partnerships.Demonstrated ability to supervise and lead a team effectively.Proven experience working with individuals with developmental disabilities or related populations.Excellent relationship management skills with the ability to build rapport with diverse individuals.Excellent communication skills both verbal and written.Education and experience qualifications relevant to the job responsibilities assigned to the staff and the primary disability of persons served by the program, including a valid degree and transcript, or a current license, registration, or certification, when a degree or licensure, registration, or certification is required by this chapter or in the support plan or support plan addendum.Demonstrated competency in the orientation and training areas required under this chapter, and when applicable, completion of continuing education required to maintain professional licensure, registration or certification requirements.Competency in these areas is determined by the license holder through knowledge testing or observed skill assessment conducted by the trainer or instructor or by an individual who has been previously deemed competent by the trainer or instructor in the area being assessed.Except for a license holder who is the sole direct support staff, periodic performance evaluations completed by the license holder of the support staff persons ability to perform the job functions based on direct observation.A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.Associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.A diploma in a field related to human services from an accredited post-secondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.Minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).Benefits :
