Job Summary: We are seeking a skilled Registered Nurse (RN) to join our team as a Home Health Nurse (RN HH). In this role, you will deliver healthcare services to patients in their residences, working closely with them and their families to develop personalized care plans.
Duties and Responsibilities: The successful candidate will be responsible for assessing patient conditions, developing care plans, charting observations, administering medication, dressing wounds, and educating patients and their families about care. They will also coordinate with other medical professionals involved in patient care.
Requirements: To be considered for this position, you must have at least 1 year of recent acute care experience in specialty. You must also have an active RN license and excellent communication skills.
