Crossroads Programs, Inc. an Apis Affiliate, Inc. is seeking a highly motivated and compassionate individual to join our team as a Case Manager in Cinnaminson, NJ. The ideal candidate is professionally passionate about making a positive impact on the lives of adolescent mothers between the ages of 14 and 21 who are experiencing homelessness or at risk of becoming homeless.
About Capable Adolescent Mother's Home:
Under the direction of the Program Manager, the Case Manager is responsible for the assessment of referrals and case management from admissions to discharge/follow-up; The Case Manager also serves as an advocate for consumers and liaisons with community providers, schools, DCP&P and courts. He/she is responsible for group facilitation, staff and on-call supervision.

Qualifications:
- Bachelor's degree in social work or a related field and two years professional experience with at-risk adolescents and/or families
