Crossroads Programs, Inc. an Apis Affiliate, Inc. is seeking an experienced Clinical Case Manager in Willingboro, NJ . Summary of Essential Duties :
Under the direction of the Program Director, the Clinical Case Manager manages a caseload of between 10 and 12 youth, working with both the youth and the treatment (therapeutic foster care) parents. The case Manager will meet weekly with all the youth in the home. Some visits may occur at the office. They attend Individual Service meetings and collaborate and communicate with all team members. The Clinical Case Manager also supports treatment parents through all placement phases, including support and on-the-job training in behavior management techniques. Additionally, manages crises as needed, monitors treatment homes to ensure compliance with licensure requirements, maintains relationships with funding sources, and represents the program to the community.

Must be 21 years of age and have a Bachelor’s degree in social work / human services, with (3) three years’ experience with at-risk youth or an unlicensed Master’s level practitioner with one (1) year of related experience.
