Job Summary:
The Case Manager will provide comprehensive case management services to individuals living with HIV/AIDS within the prison system. This role involves working closely with incarcerated individuals to address their medical, emotional, and social needs, advocating for proper HIV care, treatment, and resources, and helping them navigate their reentry into society upon release. The ideal candidate will have strong knowledge of HIV/AIDS care, the prison system, and a passion for improving the quality of life for people living with HIV.
Key Responsibilities:
- Client Advocacy & Support: Provide individualized case management to incarcerated clients living with HIV/AIDS, ensuring they have access to proper medical care, medications, mental health support, and other necessary services.
- Care Coordination: Work closely with healthcare providers, correctional staff, and external organizations to coordinate treatment plans, medication regimens, and health services.
- Assessment & Planning: Conduct comprehensive needs assessments with clients to identify their health, psychological, and social service needs. Develop individualized care plans that address these needs.
- Education & Empowerment: Educate clients about HIV/AIDS, treatment options, harm reduction strategies, and preventive care. Empower clients to take an active role in managing their health.
- Crisis Intervention: Provide crisis intervention and emotional support to clients dealing with health issues, stigma, discrimination, or trauma.
- Documentation & Reporting: Maintain accurate and up-to-date case records and documentation on client interactions, care plans, and progress. Ensure confidentiality and compliance with relevant policies and regulations.
- Linkage to Services: Assist clients with navigating external community-based services available upon release, including housing, employment, mental health counseling, substance abuse treatment, and social services.
- Collaboration: Collaborate with prison healthcare teams, social workers, community partners, and legal services to ensure clients receive holistic support.
- Support Reentry: Assist clients in preparing for their release from prison, ensuring continuity of care, helping with reintegration into society, and linking them to relevant post-release services, such as healthcare, housing, and employment.
- Advocacy: Advocate for clients’ rights and needs within the prison system, ensuring they receive the services and treatment they are entitled to.
- Bachelor’s degree in Social Work, Psychology, Public Health, a related field, four years related experience and/or training; or equivalent combination of education and experience.
- Previous experience in case management, healthcare, or working with populations affected by HIV/AIDS, preferably in a correctional or institutional setting.
- Knowledge of HIV/AIDS care, treatment protocols, and harm reduction strategies.
- Understanding of the criminal justice system and challenges faced by incarcerated individuals with chronic health conditions.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently, manage a caseload, and handle sensitive and confidential information.
- Cultural sensitivity and the ability to work with diverse populations, including individuals from various racial, ethnic, and socioeconomic backgrounds.
- Knowledge of community resources and services for individuals with HIV/AIDS, both during incarceration and post-release.
- Crisis intervention skills and the ability to manage challenging behaviors in a correctional setting.
- Strong organizational skills and attention to detail.
- Work in NJDOC facilities.
- Occasional travel to external community-based organizations or healthcare facilities
- Potential for irregular hours or on-call shifts, depending on facility needs
