Job description
:

This position provides intake, evaluation, referral, and monitoring services for children birth to three years of age who are being considered for/or enrolled in the Early Intervention Program.

Essential Job Functions:

  • Conduct intakes with children(s) and families. Schedule evaluations for eligibility as required.

  • Develops and coordinates Individualized Family Services Plans.

  • Monitors services provided to families.

  • Maintains records and documentation on all services provided via the Pelican system.

  • Screens and/or coordinates services to assess children’s developmental needs.

  • Ensures children and families receive appropriate services within the least restrictive environments and regulated timeframes.


Additional Primary Job Duties:

  • Interacts with children and families, external agencies, government departments, and outside service providers in regard to daily work duties.

  • Advises parents of their rights and procedural safeguards in regards to Early Intervention services.

  • Serves as an advocate for children and parents.

  • Promotes safety within the family’s natural environment.

  • Facilitates meetings with consumers, families, providers, etc. as required.


Other Duties of the Job:

  • Attends meetings, trainings, and in-services as required by regulation.

  • Operates computer and software programs and other office equipment as necessary to carry out job duties. Uses laptop and/or MIFI regularly at meetings and off-site work.

  • Performs other job related duties and support services as required.

  • Travels frequently to agencies, individuals’ homes, or to other job-related duties.

  • Some evening work (attendance at appointments, meetings, etc.) is involved in the performance of the job.

MET’s:

Six months of experience as a Human Services Case Manager; OR A bachelor's degree with a social welfare major; OR A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social services agency; OR Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.

Selection Criteria:

  • Meet the minimum experience and training for the job.
  • Meritorious service: defined as the absence of any discipline above the level of written reprimand during the 12-months preceding the closing date of the posting.
