The Cumberland County Department of Social Services (DSS) and the Cumberland County Department of Public Health (CCDPH) have received funding to implement an initiative which focuses on primary prevention by utilizing a whole person/family approach and addressing social determinants of health (SDOH). The initiative includes multi-disciplinary Case Management and Care Coordination teams responsive to community referrals to provide care coordination and case management services to address social determinants of health, address social needs, improve health behaviors and health outcomes, improve health equity, and reduce health disparities, reduce preventable hospital stays, and prevent and reduce child abuse and neglect.The primary purpose of the position is to provide clerical support in implementing WIC Program services.
EXAMPLE OF DUTIES
Duties for this position include, but are not limited to: Requires daily contact with co-workers, local agency staff, Connected Care participants/clients and their families/caregivers.Periodic contact is required with the area health and human service providers and their staff, staff members in other county departments, community agencies and the general public. Provide information to the public about the Connected Care program and provides information (brochures, flyers, etc.) about community resources. Review the eligibility criteria, residence, category, income, and medical/nutritional risk for Connected Care participants enrolled in WIC. Explain the rules and guidelines for the Connected Care Program. Support Connected Care staff in making needed referrals identified during screening/assessment to other agency divisions/programs medical provider offices and other area agencies/groups (follow up phone calls, emailing documents, faxing documents, etc.). Responsible for submitting referrals into NCCARE360. Schedule or reschedule WIC appointments for Connected Care clients. Employee is subject to be called upon anytime to help the agency respond to a public health emergency which may include, but is not limited to, outbreak investigations, staffing mass dispensing or immunization clinics, providing disaster relief, or other incidents (man-made or natural and intentional or non-intentional) as they arise, or any public health response requested by the Health Director.

Significant knowledge of office or work unit procedures, methods and practices.Significant knowledge of and ability to use correct grammar, vocabulary, spelling and office terminology to compose and/or proofread correspondence, reports and other materials.Significant knowledge of accounting procedures, mathematics and their application in the work environment. Ability to learn independently and apply laws and departmental rules and regulations covering programs and services.Ability to review and process information to determine conclusions, actions or compliance with applicable laws, rules and regulations.Ability to apply a variety of work-related formulas or mathematical calculations.Ability to record, compile, summarize and perform basic analysis of data.Ability to independently work with people with courtesy and tact in performing public contact duties that may be sensitive in nature.May require ability to supervise and coordinate the work of other staff, student workers or volunteers.Must have or obtain and maintain a valid North Carolina driver's license with an acceptable driving record.
