Job Description
- Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Perform home visits and attend periodic office meetings.
- Ensure reliable transportation, a current driver's license, and agency-required liability insurance.
- Service an average of 5-8 patients per day.
- Complete various types of cases such as Start of Care, Wound Care, and Follow-Up Visits.
- Work with a territory mileage radius up to 75 miles from the home clinic.
- Address on-call requirements, which vary per location.
- Use proficiency in HomeCare HomeBase EMR with recent experience.
- Complete all documentation within 24 hours of the patient appointment.
- Attend to on-call rotations as required.
- Be open to traveling and working in different locations as needed.
- Maintain productivity points with a weekly expectation of 30 points.
- Participate in weekly meetings and complete tasks as assigned by the Clinical Manager.
