What are the responsibilities and job description for the Health Navigator position at Dallas County?
This position is responsible for assisting clients with referrals to appropriate health services programs and resources.
- Screens and assesses client needs and connects clients to appropriate resources; provides in-home and other on-site visits.
- Provides support to clients and communities in the development of communication skills and risk reduction behaviors.
- Communicates with clients and service provider partners to identify issues and provide solutions.
- Coordinates with other department staff to review cases with supervisor, to manage a sustainable caseload, and discuss complex client needs.
- Researches and identifies internal and external resources to meet client needs.
- Provides information to clients regarding program eligibility requirements.
- Collaborates with medical offices, nurses, and community partners to provide services to clients.
- Attends community events such as health fairs to provide program information to the general public.
- Provides English to Spanish and Spanish to English interpretation and translation services as assigned.
- Participates in continuing education.
- Performs related duties.
EDUCATION/EXPERIENCE
- High School Diploma or equivalent.
- One to two years relevant experience in Social Work, Community/Public Health or related field.
KNOWLEDGE/SKILLS/ABILITIES
- Knowledge of program eligibility requirements.
- Knowledge of community resources.
- Knowledge of case plan management principles.
- Knowledge of computers and job-related software programs.
- Skill in problem solving.
- Skill in prioritizing and planning.
- Skill in interpersonal relations.
- Skill in oral and written communication.
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
- The work is typically performed while sitting at a desk or table. The employee occasionally lifts light objects and distinguishes between shades of color.
- The work is typically performed in an office, in patient homes, in school facilities, in other public facilities, and outdoors. The employee is exposed to contagious or infectious diseases and work requires the use of protective devices such as masks, goggles, gloves, etc.
- Eligible to telecommute with the approval of the Director of Health.
