•Responsible for the coordination and implementation of case management strategies pursuant to the Case Management Plan.
•Plans and coordinates care of the patient from hospitalization through discharge.
•Responsible for authorization of appropriate services for medical necessity, continued stay and appeals.
•Participates in performance improvement activities.
Qualifications
•Current Indiana RN licensure
•2 or more years of case management experience preferred
•Basic computer knowledge
•Knowledge of financial and/or statistical analysis
•Strong clinical, written and verbal skills
Job Type: Full-time
Pay: $62,000.00 - $80,000.00 per year
