SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
Provide case management and assessment to a crisis population in both an on-site and / or community setting; assure coordinated service delivery and successful client linkage with other programs and facilities; related duties as assigned.
Essential Job Functions :
1. Meet with consumers as assigned to develop and implement Behavior Support Plans (BSP).
2. Regularly review and revise plans with input from key persons such as the consumer, Interdisciplinary Team, family, and Beacon Specialized Living New Jersey staff.
3. Train staff directly working with the consumer to implement the BSP; may model interventions and directly assist during challenging behaviors.
4. Compile internal and external reports as required; analyze data and make recommendations.
5. Act as a team member addressing significant behavioral and / or medical changes in consumers.
6. Be available by phone for emergencies and assist in person as needed.
7. Maintain comprehensive notes and tracking records as required.
8. Participate in team meetings, agency staff meetings, and required in-service training sessions; regularly communicate with the supervisor.
9. Stay current on relevant trends, service delivery requirements, and strategic issues; recommend service improvements to maintain the highest quality of care.
10. Ensure program records are maintained and reports are submitted on time, including time and attendance and electronic case records; enter data into the electronic consumer database daily.
CLIENT'S REQUIRED EXPERIENCE & EDUCATION
BA in Mental Health or a related field.
1 year of post-degree relevant work experience.
Strong organizational, leadership, and interpersonal skills.
Proficiency in working with computers and necessary software.
Most clients require vaccination with booster (exemptions may be accepted).
