Responsibilities
- Conduct outreach activities to engage community members and identify health needs.
- Collect data on community health trends and report findings to relevant stakeholders.
- Educate individuals about available healthcare services, including Medicaid options.
- Work closely with the TORCH Program and manage referrals from the UNITE US platform.
- Provide support in navigating social services, including addiction counseling resources.
- Utilize motivational interviewing techniques to encourage behavior change and promote healthy lifestyles.
- Assist clients in understanding medical terminology related to their health conditions.
- Organize and lead workshops or presentations on public health topics.
- Collaborate with healthcare providers, social workers, and public health officials to coordinate care and services.
- Maintain accurate records of client interactions and follow-up actions.
Qualifications
- Experience or education in public health, social work, or a related field is preferred.
- Knowledge of medical terminology and healthcare systems, particularly Medicare.
- Strong skills in data collection and analysis to assess community needs effectively.
- Excellent public speaking abilities for conducting educational sessions and workshops.
- Familiarity with addiction counseling practices is a plus.
- Ability to engage with diverse populations with empathy and cultural competence.
- Proven experience in motivational interviewing techniques is desirable.
- Strong organizational skills with attention to detail in record keeping.
