Supports and strengthens the link between community-based organizations, businesses, faith communities and philanthropic groups to schools, communities and families. Supports the systems-driven approach to case management for the multi-generation services offered to participants out of the community center. Responsibilities include creating a welcoming and safe environment for all community center participants by building positive relationships; partnering with participants to identify and work towards their long-term goals for family stability; connecting community members with wraparound services to support them in building skills and accessing resources; and utilizing community center practices, procedures and data systems to deliver high-quality customer service and demonstrate program outcomes.

Serves as a community center team member, providing program support for multi-generation services for adults and youth with engagement in long-term sustainable employment and other individualized services.Serves as a case manager of community center students, following systems and structures to ensure participants have access to post-secondary education or long-term employment; supports participant caseload for case management of follow-up services to ensure community center student and family success in achieving self-sufficiency goals; performs daily data collection and program implementation for community center participants.Completes community center intake, follow-up, and satisfaction survey processes with participants.Encourages and accommodates a variety of learning and participation styles, building on staff and community members' strengths; draws upon the experiences of participants or collaborators to include diverse perspectives in any given intervention and positively collaborates with the community center and school staff members to ensure high-quality delivery of services.Works collaboratively with department staff to implement research-based systems to serve families with high-quality customer service and individualized support for family and student success.Supports systems for quality control aligned with grant compliance for community center data collection and operations; supports daily operations and program support of the community center, including daily phone calls and responding to voicemails; assists walk-in participants and scheduling appointments and leads basic needs services for community center students and participants.Leads special events or new project initiatives to maximize center capacity and to further engage participants in services; supports the ongoing expansion of services offered at the community centers and leads outreach within regional communities and collaborate with other department staff to increase awareness of services offered at the community centers.Works collaboratively with other community center staff and between community centers supporting the implementation of researched-based systems across the community centers to ensure high-quality programming throughout Denver.Meets regularly with the community center team to engage in professional development and to support best practices for the engagement of participants; engages in skills training across the department to ensure quality control and efficiency when working with grant-driven programs.What You'll Need :
