Job Summary:
The Intake Coordinator plays a critical role in ensuring the efficient and accurate processing of medical information for patients entering Home Health care services. This position requires close collaboration with healthcare professionals, patients, and their families to collect, organize, and maintain all necessary documentation in a timely manner. Strong organizational skills, attention to detail, and a solid understanding of medical terminology and procedures are essential. The Intake Coordinator is instrumental in facilitating a smooth transition from hospital or clinic to home care.
Responsibilities:
- Manage and complete the intake process for new patients, ensuring the accurate and timely collection of medical information and authorizations.
- Communicate effectively with healthcare professionals, patients, and families to gather relevant medical, insurance, and demographic information.
- Conduct thorough assessments of patient conditions and medical needs to determine appropriate services and resources.
- Verify insurance coverage and benefits, obtaining necessary authorizations for Home Health services.
- Collaborate with healthcare providers and care teams to ensure the correct assignment of Home Health services, treatments, and equipment.
- Request and acquire medical records, test results, discharge summaries, and other essential documentation from hospitals, clinics, and healthcare facilities.
- Enter and maintain accurate, up-to-date patient information in electronic medical records (EMR) and other data management systems.
- Review clinical information and records to identify and resolve discrepancies, missing documentation, or potential issues.
- Ensure compliance with privacy and confidentiality regulations, including HIPAA, when handling patient information.
- Provide excellent customer service by addressing patient and family concerns regarding the intake process and available services.
- Assist with insurance authorization and scheduling as needed.
Qualifications:
- High school diploma or equivalent required; associate’s or bachelor’s degree in a healthcare-related field preferred.
- Minimum of two years of experience in a healthcare setting, preferably in Home Health medical intake or scheduling.
- Strong knowledge of medical terminology, procedures, and healthcare regulations.
- Proficiency in electronic medical records (EMR) and data management systems.
- Exceptional attention to detail and accuracy in handling documentation and data.
- Excellent organizational skills with the ability to manage multiple tasks efficiently.
- Strong interpersonal and communication skills, with the ability to establish rapport with patients, families, and healthcare professionals.
- Ability to maintain confidentiality and handle sensitive patient information appropriately.
- Familiarity with insurance verification and authorization processes preferred.
- Ability to work independently and collaboratively within a team.
- Flexibility to adapt to changing priorities and schedules, including occasional evening or weekend shifts as needed.
