Job Summary:
The Hospice Intake Specialist plays a critical role in managing the referral and admission process for patients seeking hospice care. This position is responsible for gathering and verifying patient information, coordinating with healthcare providers, and ensuring that patients are admitted to hospice services efficiently and compassionately. The Hospice Intake Specialist is often the first point of contact for patients, families, and referring providers.
Key Responsibilities:
-Referral Processing: Receive and process hospice referrals from healthcare providers, hospitals, and family members, ensuring all necessary documentation is obtained.
- Patient and Family Communication: Provide compassionate and clear communication to patients and families about the hospice admission process, services, and eligibility requirements.
- Data Entry: Accurately enter patient information into the electronic medical record (EMR) system and maintain up-to-date documentation for each referral.
- Insurance Verification: Verify patients' insurance coverage, benefits, and eligibility for hospice services, working with billing departments as needed.
- Coordination of Care: Collaborate with clinical teams, social workers, and external healthcare providers to facilitate a smooth and timely admission process.
- Regulatory Compliance: Ensure compliance with Medicare, Medicaid, and state-specific regulations regarding hospice care admissions.
- Follow-Up: Conduct follow-up with referral sources, patients, and families to provide updates on the intake and admission process.
- Documentation: Ensure that all referral and admission paperwork is completed accurately and submitted in a timely manner.
Qualifications:
- Education: High school diploma or equivalent required; Associate's or Bachelor’s degree in healthcare or a related field preferred.
- Experience: Minimum of 1-2 years of experience in healthcare intake, hospice, home health, or a related field.
- Skills:
- Strong organizational and time management skills.
- Excellent communication and interpersonal skills.
- Proficiency in EMR systems and healthcare databases.
- Ability to handle sensitive and confidential information with discretion.
- Knowledge of hospice services, Medicare/Medicaid regulations, and insurance verification processes (preferred).
Working Conditions:
- This position works in an office environment Monday to Friday 8 to 5
- Standard business hours with potential after-hours on-call responsibilities depending on organizational needs.
