Overview
Discovery Behavioral Healthcare is seeking a dedicated and compassionate Recovery Case Manager to join our team; to provide intensive case management, care coordination and advocacy services that promote client stability within the community, along with rehabilitation services and/or activities for our clients.
Responsibilities
- Assistance with meeting basic life needs, developing and maintaining interpersonal relationships, improving financial stability, and improving housing access.
- Help with, normative function within primary setting (school, work, home, or placement)
- Teach skills such as problem-solving, goal-setting, social-skills, money management, peer-relationships, symptom management, and employment skills.
- Conduct behavioral counseling to assist client in following plans designed to assist in symptom reduction or resolution.
- Develop, update, and monitor individualized care/treatment plans in collaboration with consumers, their support systems, other relevant persons and care teams as appropriate.
- Provide Family support and education.
- Assist with group activities or treatment,
- Perform duties collaboratively with clinical, medical and administrative team
- Compassionately engage, interact and communicate with persons experiencing mental illness, emotional disturbance or chemical dependence in a positive and pro-social manner while maintaining appropriate personal and professional boundaries.
- Actively coordinate with other community agencies and systems.
- Other duties as assigned.
Requirements
- Bachelor's degree in Social Work, Psychology, or a related field is preferred.
- Must hold a current (or in process of getting) Washington State Agency Affiliated Counselor (AAC)
- Valid Washington State Driver’s License
- Computer competency including Microsoft Windows, Word, and other software, with the ability to learn EMR software programs.
Job Type: Full-time
Pay: $25.86 - $27.64 per hour
