We are seeking a qualified person to be part of our interdisciplinary team that provides social service case management to short term and long term residents.

Working as a resident and family social worker, you will be responsible for:

• advocating for resident rights
• evaluating and meeting resident psycho-social needs
• coordinating discharge plans
• arranging for services such as home care or hospice
• developing a comprehensive assessment and resident-based care plans
• exercising independent judgment, making sound decisions, and exhibiting flexibility when needed
• ensuring and providing therapeutic interventions to assist residents in coping with their transition and adjustment to their placement
• serving as liaison between residents, families, the facility, and outside agencies
• providing oversight for our long-term care residents psycho/social needs

Required Qualifications

• bachelor's degree in social work or in a human services field including but not limited to: gerontology, sociology, rehabilitation, counseling, psychology.
• excellent communication and listening skills
• ability to organize and prioritize effectively
• good problem solving skills
• preferred knowledge of long-term health care federal and state laws, rules and regulations
