What are the responsibilities and job description for the Adult Outpatient Services Community Support Specialist position at DuPage County Health Department?
The DuPage County Health Department is proud to announce an opportunity to work in our Behavioral Health Services - Adult Outpatient Program. Our Adult Outpatient program provides treatment to individuals with serious mental illness through individual & group counseling, community support, as well as psychiatric treatment. Our program is part of a larger behavioral health program that includes crisis services, adult intensive services, residential services, substance use treatment, and justice programs.
This position will work out of our Central Public Health Center in Wheaton, IL. Typical schedule is Monday through Friday, 8:00 AM until 4:30 PM, 4-days of the week, and 10:30 AM until 7:00 PM one day per week.

- Manages a case load and maintains regular contact with clients outside of the office, homebased and community based to meet client need and engage at risk clients
- Acts as client advocate to facilitate access to resources and services
- Works collaboratively with providers to achieve client goals
- Develops and maintains client treatment plans
- Provides intervention to support client self-management of their illness
- Demonstrate the ability to engage adults and families
- Acts as a liaison between healthcare resources and clients
- Participates in client staffing and team meetings
- Demonstrates independent judgment and discretion in making position related decisions
- Meets established productivity standards
- Performs concurrent documentation
- Maintains collaborative relationships with co-workers to effectively support clients
- Participates in emergency activities as assigned
- Maintains required training, licensure and/or certification
- Maintains confidentiality of privileged information and adheres to client privacy laws
- Demonstrates sensitivity and understanding of other ethnic groups and cultures
- Adheres to department guidelines for attendance and punctuality
- Provides skill building and case management supports to clients in the community
- Works during assigned business hours, allowing for flexibility in the schedule to include days, evenings
Completion of a Bachelor’s Degree in Psychology, Social Work or related field, and one year of behavioral health experience; or an equivalent combination of training and experience.
ADDITIONAL REQUIREMENTS:
Designated positions may be required to possess a valid Illinois Driver’s License and a motor vehicle background check.
Salary : $44,400 - $54,400
