Essential Job Functions:
- Build Strategic Relationships: Establish and maintain strong partnerships with hospital case managers, discharge planners, physicians, and other healthcare providers to drive referrals and increase census.
- Coordinate Admissions: Collaborate with referral sources to screen prospective residents, ensuring the facility can meet their care needs and facilitating efficient admissions.
- Develop Partnership Programs: Identify opportunities to partner with hospitals, physician groups, and managed care organizations to build referral pipelines and maintain census goals.
- Conduct Facility Tours: Host on-site tours for hospital staff, families, and prospective residents to highlight the facility’s capabilities and services.
- Streamline Admissions Process: Act as a liaison between hospital referral sources, families, and the admissions team to ensure a seamless and timely admission process.
- Data-Driven Planning: Track and analyze referral data, hospital discharges, and admissions trends to identify growth opportunities and implement targeted outreach strategies.
- Collaborate with Facility Teams: Work closely with the nursing, social services, and admissions teams to ensure a high standard of care and smooth transition for incoming residents.
- Educate Referral Sources: Regularly engage with hospital case managers and physicians to provide updates on facility programs, specialty services, and success stories.
- Lead Census Initiatives: Implement strategies to achieve census goals, including proactive outreach to potential referral sources and follow-ups with pending admissions.
- Foster Community Awareness: Represent the facility in the community and at healthcare events, showcasing Dynasty Healthcare as a preferred provider for skilled nursing and rehabilitation.
- Bachelor’s degree in healthcare, marketing, or a related field preferred.
- Minimum of 2 years of experience in healthcare business development, with a focus on skilled nursing or post-acute care.
- Proven track record of census growth through hospital or healthcare partnerships.
- Strong relationship-building skills, with a network of contacts in hospitals, physician offices, and other referral sources.
- Excellent communication and presentation skills, both written and verbal.
- Ability to analyze data to develop actionable strategies for census building.
- Self-motivated and able to work independently while also collaborating effectively with interdisciplinary teams.
- Familiarity with the skilled nursing regulatory environment and the needs of hospital discharge planners.
- Respect for resident rights, confidentiality, and compliance with all regulations and standards.
- Must pass a criminal background check and meet other hiring requirements.
