Position Overview:
The Care Manager will be responsible for providing comprehensive case management services to members in the Adult Foster Care (AFC) program. The Care Manager will ensure compliance with MassHealth regulations while coordinating care, advocating for members, and supporting the goals of Sarah Care Adult Foster Care. This role involves assessing, developing, implementing, and monitoring care plans, as well as collaborating with caregivers, family members, and other service providers to optimize the care and well-being of AFC members.

Key Responsibilities:

  • Initial Assessment & Care Plan Development:

    • Conduct thorough initial assessments of each AFC member’s medical, emotional, and social needs, including a review of health history, functional status, and personal preferences.
    • Develop individualized, person-centered care plans in compliance with MassHealth regulations, ensuring that plans are updated regularly to reflect changing needs.
  • Case Management & Monitoring:

    • Coordinate and monitor services for each member, ensuring that care plans are being followed and updated as necessary.
    • Provide ongoing case management, including regular home visits to assess the well-being of members and to make adjustments to care plans as needed.
    • Maintain accurate and up-to-date documentation for each member, ensuring compliance with MassHealth and AFC program regulations.
  • Caregiver Support & Training:

    • Offer ongoing training and support to caregivers in the Adult Foster Care program to ensure they have the knowledge and skills necessary to meet the needs of members.
    • Provide guidance and support to caregivers on issues related to care delivery, crisis intervention, and member engagement.
  • Compliance & Reporting:

    • Ensure that all services provided are in compliance with MassHealth and other regulatory requirements for Adult Foster Care.
    • Maintain accurate records of all assessments, care plans, and progress notes, ensuring compliance with state and federal regulations.
    • Prepare for and assist with audits and regulatory reviews as required by MassHealth and other relevant bodies.
  • Collaboration & Communication:

    • Serve as the primary point of contact between members, caregivers, and external healthcare providers, facilitating communication to ensure continuity of care.
    • Collaborate with interdisciplinary teams, including physicians, social workers, and other professionals, to ensure that care plans are comprehensive and integrated.
  • Advocacy & Quality Improvement:

    • Advocate for members’ rights and well-being, ensuring they receive the highest standard of care and services.
    • Actively participate in the development and implementation of quality improvement initiatives aimed at enhancing the AFC program’s effectiveness and member satisfaction.

Qualifications:

  • Bachelor’s degree in Social Work or a related field.
  • At least one year of experience working with elderly and disabled populations, with a preference for experience in case management or care coordination in long-term care or home and community-based settings.
  • Knowledge of MassHealth regulations, particularly those related to the Adult Foster Care program.
  • Ability to perform comprehensive assessments and develop individualized care plans.
  • Strong organizational skills with the ability to manage multiple cases and priorities effectively.
  • Excellent communication and interpersonal skills, with the ability to build relationships with members, caregivers, and other providers.
  • Ability to work independently as well as part of an interdisciplinary team.
  • Proficiency with electronic health records (EHR) and other case management software.
  • Valid driver’s license and reliable transportation for home visits.
