Position Description
Position Title: Intake Coordinator
Status: Part-time, exempt
Reports to: Associate Director or Designee
Supervises: N/A
Summary: Facilitates access to services for clients referred to ECMHP programs.
Essential Duties and Responsibilities:
· Receives and responds to referrals for CFB, CTP, PCOC and Solano P25 program services, made by phone, email, fax, or other referral avenues. Handles referrals to other programs as designated
· Screens for and records all necessary information to determine eligibility, need, and appropriateness for program participation
· Gathers and documents information to assess clinical need, including main concern, impact on current functioning and risk, usually by phone
· Assists referred families with resources and referrals for immediate needs; provides basic case management such as linkage and advocacy if required
· Develops and maintains waitlists and databases for referrals for each program, tracks volume of referrals, intakes and case assignments
· Supports Clinical Leadership and Management in triage to appropriate services and level of care within the agency
· Verifies and records Medi-Cal or program eligibility and supports families navigating barriers to eligibility
· May on occasion perform chart auditing functions for quality assurance purposes
· Provides clinical maintenance and temporary case management for clients awaiting service assignment, in accordance with Medi-Cal, county and state requirements
· Completes agency paperwork required to open Medi-Cal eligible cases; documents clinical activities for Medi-Cal billing
· Other duties as assigned
Minimum Requirements:
1. Master’s degree in social welfare, psychology, counseling, or a related field; alternatively, a bachelor’s degree with a minimum of 12 Child Development Units
2. One year of experience doing case management or family advocacy for families with young children.
3. Excellent organizational, communication and listening skills.
4. Fluency in both English and Spanish.
5. Must submit to fingerprinting and be cleared by the California Department of Justice and the FBI for criminal and child abuse background checks.
Physical Requirements:
Employees must be able to stand, walk, sit, bend, and crouch without physical limitations; and must be able to work effectively in a high-noise, rapid-pace environment.
Preferred Qualifications:
1. Thorough knowledge of resources available for children and families throughout Contra Costa and Solano Counties.
2. Licensed or eligible for licensure by either the California Board of Behavioral Science or the California Board of Psychology.
